Hospice Nurse Practitioner – Saint Mary’s Hospice of Northern Nevada

Saint Mary’s Hospice of Northern Nevada, part of the LHC Group and the Optum family, is seeking an experienced Hospice Nurse Practitioner to support patients through palliative, symptom management, and end‑of‑life care. This role focuses on providing compassionate, patient‑centered care while collaborating closely with physicians and interdisciplinary teams.

Role Highlights:

• Conduct face‑to‑face visits with hospice patients and report clinical findings to the Hospice Medical Director
• Provide palliative care consultations, symptom management, and supportive care
• Collaborate with referring physicians, interdisciplinary care teams, patients, and families
• Initiate and coordinate individualized hospice plans of care
• Educate and support patients and families on self‑care when appropriate
• Maintain timely, accurate clinical documentation and communicate patient needs

Required Qualifications:

• Active, unrestricted NP license in the state of residence
• National NP or CNS certification (ANCC, NBCHPN, or AANP)
• CPR certification
• 5+ years of nursing experience
• Valid driver’s license and reliable transportation

Preferred Experience:

• Hospice, palliative care, oncology, geriatrics, or ICU experience
• Experience working within an interdisciplinary team
• Palliative Care ARNP background
